Fillers are used to restore lost volume all over the body, mainly due to aging. Filler injection is a method that is easy and quick if performed by a specialist physician, which almost needs no recovery.

Skin Fillers: A Brief History

Fat was the first thing used as a filler. By the end of the 19th century, fat had been used from other parts of the body to fill the volume under the skin in traumatic patients. In fact, fat transfer has been the most common filling method used in the 20th century. In the 1980s, collagen taken from cows became the main filler. However, because it was an external material, collagen caused allergic reactions and had short-term effects.

In the 1990s, the second generation, who were starting to age, and as a result, seeking a fast and natural process to keep their appearance good, were able to create a great revolution in the field of beauty fillers. Many materials that could be used as fillers were tested. Hyaluronic acid fillers, which are nowadays the “gold standard” for soft tissue augmentation, were made during this time. Nowadays, numerous and varied products are being produced at a fast pace in the field, each claiming to be the best product.

Fillers can be divided into two main categories: permanent and temporary. Each of them has its advantages and disadvantages. Temporary fillers are generally less risky, look more natural and if you get an undesirable result, it is fortunately not permanent, and you can get rid of it even if it is a little far from your ideal. However, these fillers will cost you more in the long run, given the need for repetition. It is best to talk to your doctor about the choice between these two methods before injection.

Fillers can be used in different parts of the face. Fillers can be used to remove more superficial lines, such as wrinkles around the nose, lines around the mouth, and deep wrinkles such as frown lines. It is also used to restore lost volume (due to aging) in the cheeks and under the eyes. Fillers are also used to increase lip volume.

A relatively newer use of fillers is to fill in other areas of the face: to fill the chin or cheekbones, or even a more beautiful nose.

Skin fillers: 10 Important things to know

Skin fillers are an inseparable part of anti-aging cosmetic medicine. Millions of treatments with retiline have been performed since the FDA approved it for the first time five years ago.

Nowadays, various brands of fillers are available to us, such as Giodermi, Bluetro and Theocell. There are many misconceptions and confusions about hyaluronic acid fillers. Here are three important things to know before you take a filler injection.

1-Fillers return the lost volume and lift the skin

Fillers are gels made of modified hyaluronic acid molecules, and have varying degrees of consistency, hardness, and flexibility. When the fillers are injected into the skin, they restore the lost volume, for example, they fill the cheeks. This action will make the skin stretch, resulting in a younger look. Since bone and fat are both replaced with fillers, the effect of years of aging usually disappears with one injection, and if the injection is done correctly, the result will be a healthier, more beautiful appearance.

2- Fillers correct the texture of the skin

Hyaluronic acid absorbs several tons of water – about 5 times its own volume – so, when injected into the skin, it fills the skin and moisturize it. They can also improve the texture by reducing pores and brightening the skin. The popular treatment of  booster injections for skin is based on this. Wrinkles, superficial lines, pores, skin texture and color are corrected by this injection.

3- Fillers shape face parts – but not completely

Skin fillers are a good alternative to cosmetic facial plastic surgery. Parts of the face that can be filled with fillers include the nose, chin, cheeks and forehead.

However, fillers do not completely replace plastic surgery. For example, if you have a very high nose with a plumped tip – a rhinoplasty can work well for you. It is best to consult a doctor to get your desired result to make sure if the fillers are right for you.

4- If you don’t repeat the filler injection, you will not look bad

One of the common misconceptions is that if you inject once, you can no longer stop injecting, because if you do not repeat the injection your face will look bad. After the injection, the skin fillers stimulate your body’s collagen, so even after a period of treatment, your condition will be better than before, even though the effect of the injection disappears within a year or months. But know that after repeated injections, the effect will be prolonged.

5- More isn’t always better

The biggest problem with injectable treatments today is, in many doctors’ views, the injections is too much. This has led to a common phenomenon – the pillow face – which describes the excessive puffiness of those who have over-injected. Both are to blame – sometimes patients want more and more injections or sometimes physicians encourage patients to have multiple injections for financial gain.

Fillers can also be addictive. However, the result of the filler injection follows the J-shaped curve – you get a better look when the injection is done properly, but you will get a bad look if you overdo the injection.

Keep in mind, fillers are different from a buffet, it means that more injections do not mean you will be more beautiful. The important thing is that the result of the treatment is both beautiful and natural.

6- Injections may require several different skin fillers; your doctor may suggest a different filler than what you wanted.

Doing an injection is like painting – but this time it’s going to be done on your face instead of the canvas. Just as you cannot paint an image with just one color, it is impossible to achieve the desired result with one type of filler.

Since the skin and anatomy of each part of the face are different, therefore, fillers with different physical properties (such as softness, flexibility, molding) must be “combined”. For example, I use 2 different skin fillers to treat the under eye area.

It is difficult even for doctors to know all the brands of hyaluronic acid fillers on the market today. In addition, each brand has several types of the same family. In my opinion, among the reputable brands of fillers such as Retiline, Juoderm, Theocell, Bluetro, one cannot choose a brand that has obvious superiority over others. Each brand has some products with different strengths and weaknesses – and it depends on the doctor to choose which fillers are suitable for different purposes, just like how the painter chooses its color palette.

This is why your doctor may choose a filler that is different from what you originally thought it would be because he thinks you will get a better result with that particular filler. Don’t worry about asking your doctor which one to choose and why he or she prefers that type to other types of fillers.

Fillers can last a long time

Hyaluronic acid fillers consist of hyaluronic acid molecules. They are gradually decomposed by our body’s natural hyaluronidase over time. Chemical modification through polymerization and cross-linking gives the fillers different hardness, flexibility and longevity.

As technology improves, the impact of the fillers gets longer and longer. In general, thinner fillers such as retillin are the shortest-lasting type, with a shelf life of 3 to 5 months. Harder fillers such as zhevoderm have a shelf life of up to 6 months. New second-generation fillers, such as Niovia, have a shelf life of 6 months or more. Filler life depends on many factors such as product, lifestyle, face area and even injection method. Repeated treatment increases the shelf life of the fillers.

8) Each doctor’s style of work is different

It is very important to know your doctor’s style of work and make sure you are satisfied with it. Prior to the injection, ask for pictures of before and after treatment of other patients to find out exactly what the clinician considers to be the ideal outcome.

For example, a physician may care about the naturalness of the outcome more than other physicians. It may still be a few wrinkles even after the injection, but your face is still maintained natural and no one will notice that you did an injection.

9) The physician’s expertise is as important as the type of filler

The hyaluronic acid filler is just one gel in the syringe. If it is in the hands of a professional doctor, this gel can work a miracle. In the hands of someone who has no specialty, it can cause disaster. There are reports of beauty salons that injections are given by non-specialists and claiming to use “retiline” and “juodrim” with low costs.

Remember that authentic filler brands are only given to reputable doctors; and even if registered salons can access these gels, still having an untrained injector can be very dangerous.

The product used in the injection is only part of the equation. Having a specialist physician is the most important thing to get the desired result.

2) Severe, though rare, complications can occur

Gel injection is a miracle. However, side effects can also occur. Mild and common side effects include temporary swelling and bruising. More serious and unusual side effects include infection and immune reactions.

Severe complications include skin necrosis (loss of skin) and even blindness due to abnormal artery occlusion by the gel. This is a rare, but it can be catastrophic if it does occur. Be sure to talk about the risks before injecting.
