Causes of sagging breasts
There are several causes for sagging breasts (ptosis). The most common of these are breast tissue degeneration after pregnancy, severe weight loss, removal of the breast prosthesis, or decreased skin firmness following aging.
An important point in “mastopexy” surgery or breast augmentation is the scar or surgical site, which can vary depending on the type of surgical technique.
This is very important and should be discussed with the patient before the operation, as it plays an important role in the patient’s decision to have surgery. If the sagging of the nipple is below two or three centimeters, only peri-areolar incisions can be made. If the sagging of the nipple is more than two or three centimeters, in addition to the incision around the aura, a vertical incision should be made, the beginning of which is six o’clock and the end of which is the submandibular fold.
However, reverse T-incision, which in addition to previous incisions involves horizontal incision in the crease area under the breast, is less commonly used today because of the excessive length of the scar.
Another method of “mastopexy” is suctioning the breast tissue, which, of course, can slightly increase the size of the breasts. In this method, the adipose tissue of the breasts is removed and the nipple rises as the breasts become lighter.
According to what has been said, the best choice for suctioning is for people whose bulk breast tissue is fat (not glandular tissue) and whose skin is normal. One of the advantages of this method is that the scar is less active and there is no damage to the vascular and nervous tissues of the breast. This can be especially helpful for people who have previously had surgery to reduce their mammoplasty but are still complaining about the large size of their breasts.
Complications of mastopexy
1- Nipple necrosis: One of the disappointing and undesirable side effects of “mastopexy” is necrosis (blackening) of the nipple, which can occur in five percent of people.
2 – Scar surgery: Although scar is one of the complications of surgery, but the final quality of the operation can not be fully predicted. Unfortunately, those age groups who are less sensitive to the site of the operation (Oscar) (older people with thin skin) are less likely to be operated on, and vice versa, to younger people who are more sensitive to the site of the operation (Oscar).
The place of action is more visible. It should be noted that scar surgery is more common in people with dark, thick and oily skin, and the best way to reduce this problem is to use the method that has the least surgical incision.
The use of vitamin E, compression bandages, adhesives or silicone sheets or topical injections of corticosteroids can reduce the severity of scarring.
Note: Reoperation to remove the scar is best done one year after the previous operation.
